Frequently asked questions about our services.
1. What services do you all provide?
We provide mental health services including initial psychiatric evaluations, medication management, some supportive psychotherapy, Attention Deficit Hyperactive Disorder testing, and medical marijuana evaluations.
2. Are all of your appointments conducted via Telehealth?
Yes, ALL APPOINTMENTS ARE TELEHEALTH, THIS CONVENIENCE IS FOR YOU! Let us meet you where you are! Be sure that you have a stable cellular or Wi-Fi connection and appropriate devices to conduct your appointments.
3. Do you all accept insurance?
No, we do not accept any insurance, we are a Cash Pay Only practice. We do not provide Superbills. We accept AMEX, Discover, Mastercard, and VISA.
4. What ages do you treat?
We treat all persons between the ages of 5 years and 95 years.
5. What conditions do you specialize in?
We as a team specialize in all mental health conditions, however here at EVOLVE our focus is Depression, Anxiety, Bipolar Disorder, and ADHD.
6. What should I expect in my initial visit?
You will first be greeted by one of our EVOLVE family members to verify your information and collect payment. Once this is done you will be transferred to your provider's waiting room to be seen. You will be seen by the provider and a treatment plan will be created including but not limited to medication management, testing needed, psychotherapy recommendations, and follow-up plans.
7. How often will I see my provider?
When establishing care, we want to ensure that we are hearing all of your needs and we have time to get to know one another, after your initial visit you will be scheduled for a follow-up visit within 4 weeks (typically in 2 weeks) and usually seen monthly after that until you are considered maintaining and stable with the ability to eventually be seen every 12 weeks if warranted.
8. How will I receive my prescriptions?
Prescriptions will be electronically sent to your pharmacy of choice after each appointment and you have the ability to utilize insurance at the pharmacy if you have the prescription plan.
9. How will I get labs drawn?
We utilize LabCorp, an order will electronically be sent to LabCorp on your behalf after each appointment and you will have the ability to utilize insurance at LabCorp if you have the appropriate coverage.
10. Do you all perform Genesight testing?
Yes, we do have access to order Genesight testing for those that are interested. Please note that some insurance may have certain requirements needed prior to their paying. We will direct you to Genesight for any pricing inquiries.
11. How much is it for the initial psychiatric evaluation?
Initial Psychiatric Evaluation up to 60 – 75 minutes (Adult) – 350.00 and up (Extended Supportive Therapies Provided as needed) Initial Psychiatric Evaluation up to 60 minutes (Child – Ages 5 -17) – $375.00
12. How much is it for my follow-up appointments?
Established Patient – Symptom Management – (0-15 minutes) – $150.00 Established Patient (16-30 minutes) – $200.00 Established Patient (31-45 minutes) – $250.00
13. I am in need of an urgent psychiatric evaluation. How much will this cost?
Urgent Psychiatric Evaluation up to 45 minutes (Adult) – $400.00 Urgent Psychiatric Evaluation up to 45 minutes (Child – Ages 5 -17) – $425.00
14. What if I run over my time allotted? Is there an additional charge?
Yes, $2.00 additional for every minute above the stated evaluation time
15. Is there an additional charge for ADHD Testing?
Yes, ADHD Diagnostic Test – $50.00
16. Do you all complete disability forms and document requests? Is there a fee?
Yes, Document Completion associated with your care/court/FMLA/Disability – $225.00 per hour, minimum of 1 hour per Document Requested.
17. Do you all do Medical Marijuana Evaluations? How much will it cost?
Medical Marijuana Evaluations – $150.00 Medical Marijuana Re-Evaluation – $100.00
18. Can I pay at a later time or is payment due when I see my provider?
Payment is due at the time of service unless other arrangements have been made in advance. Payment must be received prior to any prescriptions being sent to the pharmacy. EVOLVE accepts debit cards and credit cards. Overdue accounts may be referred to collection agencies as a last resort after one month of non-payment. There is a $50 charge per each returned and unpaid transaction.
